RN Per Diem - Home Based Primary Care (HBPC) Care Manager - Redding

Demonstrated clinical competencies in such areas as age-specific care, cultural assessment, and health education. An ability to effectively function autonomously, as well as a member of an interdisciplinary team Respect for cultural and spiritual values. A commitment to discipline-specific standards of practice, including:
ethical conduct, the primary care delivery model, chronic disease management, palliative care, principles of care management, a holistic framework of practice, and geriatric care skills (including infection control, equipment and environmental safety). Determine the appropriateness of patients for admission into the HBPC Program. Help develop patient treatment plans based on a comprehensive interdisciplinary assessment of the patient and the caregiver. Implement the patient's treatment plan through home visits, as needed. Review the patient's progress as a team at regular intervals, at least every 90 days, or when there is a change in the patient's condition. Assess and support the caregiver's capability to continue to provide care. Educate the patient and the caregiver. Coordinate the process of care with the other team members to maximize efficiency. Assess the patient's continuing need for HBPC care, and plan for HBPC discharge when indicated. Document all patient care activities in a timely manner (per facility policy). Identify areas for continuing education and participate in in-service training (including staying current with all required education). Participate in developing and implementing the performance improvement plan (including quality monitors). Instruct students of various disciplines in the challenges encountered in delivering health care in the home setting. Serve as a role model in the effective operation of an interdisciplinary team. Participate in the development and periodic review of the local HBPC Policy and Procedure Manual. Utilize VHA and community resources as appropriate. Participate in HBPC team meetings. The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Experience:
BSN and/or Master's degree from an accredited school of nursing recognized by the VA (Commission on Collegiate Nursing Education (CCNE) or The Accreditation Commission for Education in Nursing (ACEN). Previous Home Health/Hospice in the last five (5) years. Current proficiency in Basic Life Support (BLS) through the American Heart Association (Required prior to assuming clinical duties and must remain current -VHA Directive 11-77) Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. heavy lifting of 45 pounds and over, heavy carrying of 45 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13 to 16 to Jaeger 1-4, far vision correctable in one eye to 20/20 and to 20/40 in the other.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
