CLINICAL Establishing pharmacist-patient relationship Independently manages outpatient drug therapy of patients in assigned areas. Provides patient education (individual and group) both formally and informally in various ambulatory care settings Performs limited physical assessments Serves as drug therapeutic consultant to medical and nursing staff Serves as liaison with Pharmacy Service, Outpatient Clinics, and other related areas Reviews non-formulary drug requests (NFDRs) EDUCATION Precepts pharmacy residents, pharmacy students, as well as clinical pharmacists participating in staff development programs Prepares formal and informal presentations related to rational drug therapy for providers and clinic staff Provides orientation and training to new employees, trainees, volunteers and other assigned personnel Assists in developing, organizing and implementing new training and education concepts for PGY1 Pharmacy Residents and APPE/IPPE pharmacy students RESEARCH Designs and conducts research projects related to health care issues or drug use control issues Develops systems for documentation and justification of clinical services. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (PharmD)degree. Today the sole degree is Doctor of Pharmacy.)Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Policy. GS-13 Clinical Pharmacy Specialist. Experience. In addition to the GS-12 requirements, applicants must have 1 year of experience equivalent to the GS-12 grade level including providing evidence-based pharmaceutical care services through direct care to with patients and consultations with other providers on their team, providing direct patient care within the Primary Care Mental Health Integration (PCMHI) program, initiating, modifying or discontinuing medication therapy and providing professional training to PGY1 Pharmacy Residents, as well as training for Pharmacy clinical clerkship students from Schools/Colleges of Pharmacy. (This experience must be supported by your resume to be creditable.) Preferred
Experience:
Completion of the PGY2 Psychiatric Pharmacy Residency program or have equivalent work experience providing direct patient care to the mental health population. HIGHLY QUALIFIED DETERMINATION FOR REMOTE ASSIGNMENT CONSIDERATION:
In order to be considered for a remote duty assignment, you must have completed a PGY2 psychiatric pharmacy residency program OR you must have four years experience in the area of specialty psychiatric pharmacy practice OR you must have completed a PGY1 pharmacy residency program plus two years experience in the area of specialty psychiatric pharmacy practice. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). The work requires standing, light carrying and lifting (under 15 pounds); walking, bending, reaching above the shoulder, and ability to distinguish basic colors. The work also involves working around machinery with moving parts and working closely with others. A physical examination prior to placement is required.
